Project acronym: iPDM-GO

Integrated personalized diabetes management (iPDM) is an iterative process shown to improve outcomes, treatment satisfaction and patient adherence. iPDM-GO will build on these results and realise iPDM in Denmark, enhancing it through a digital patient assessment tool and exploring predictive modelling approaches. The digital tool will help healthcare professionals and people with type 2 diabetes pinpoint the most appropriate intervention strategies and facilitate shared decision making.
